They seem to be related to WP-Minify. Can you try disabling that plugin and see if this form works.</p> <p>The simpler form will work because it does not contain conditional logic. The more complex for probably uses conditional logic, or AJAX, and the JavaScript error prevents the form from showing because the required Gravity Forms jQuery can't run to display the form.</p> <p>Please try disabling WP-Minify first and see if that fixes things.
