&#60;li&#62;Recipient's name field&#60;br /&#62;&#60;strong&#62;Name Field&#60;/strong&#62;&#60;/li&#62;
&#60;li&#62;Recipient's e-mail field&#60;br /&#62;&#60;strong&#62;Email Field&#60;/strong&#62;&#60;/li&#62;
&#60;li&#62;Optional message from sender to recipient to be included in the e-mail body&#60;br /&#62;&#60;strong&#62;Paragraph Text Field&#60;/strong&#62;&#60;/li&#62;
&#60;li&#62;Hidden field with pre-written copy to be included in the e-mail body&#60;br /&#62;This does not need to be included as a field. After saving your new form, you'll be able to modify the notifications. This would go in the &#60;strong&#62;Autoresponder&#60;/strong&#62; message section.&#60;/li&#62;
&#60;li&#62;Custom formatted html styling for the e-mail's body&#60;br /&#62;Same deal as above.&#60;/li&#62;
&#60;li&#62;Hidden field for post title to be used in the e-mail subject and body&#60;br /&#62;&#60;strong&#62;Hidden Field&#60;/strong&#62; has a built in option to autopopulate the default value with the Post/Page title (on the advanced tab).&#60;/li&#62;

			<description>&#60;p&#62;A few threads came back matching &#34;tell-a-friend&#34;, but nothing really touched on creating a form that can be used to share individual posts via email with a friend.&#60;/p&#62;
&#60;p&#62;It seems this is possible with Gravity, but I'm not quite sure how -- just getting started with the plugin.&#60;/p&#62;
&#60;p&#62;So here's what I'd like to do, and could use some help setting up.  I imagine the form including:&#60;/p&#62;
&#60;ul&#62;
&#60;li&#62;Sender's name field&#60;/li&#62;
&#60;li&#62;Sender's e-mail field&#60;/li&#62;
&#60;li&#62;Recipient's name field&#60;/li&#62;
&#60;li&#62;Recipient's e-mail field&#60;/li&#62;
&#60;li&#62;Optional message from sender to recipient to be included in the e-mail body&#60;/li&#62;

&#60;p&#62;Here's the workflow I picture:  I like a post on a site and I want to share it with a friend.  I see a &#34;tell-a-friend&#34; form below the post and I fill it in.  I type my name, my e-mail address, my friend's name, my friend's e-mail address, and a message, &#34;this is hilarious...enjoy!&#34;.  I hit the &#34;send&#34; button and a message fades in over the form, &#34;This post has been sent to Mike (my friend). Thanks for sharing!&#34;.&#60;/p&#62;
&#60;p&#62;In my friend's inbox is an e-mail from my address with the subject, &#34;Travis wanted to share &#34;Funny freaking post&#34; with you at FunnySite.  He opens it and finds a beautiful html email that greets him with, &#34;Hey Mike, Travis liked a post called &#34;Funny freaking post&#34; on FunnySite and wanted to share it with you:&#60;/p&#62;
&#60;p&#62;&#60;a href=&#34;http://funnysite.com/funny-freaking-post&#34; rel=&#34;nofollow&#34;&#62;http://funnysite.com/funny-freaking-post&#60;/a&#62;&#60;/p&#62;
