Is it possible to add a question type with 'multiple row' text entries? It would work like the Likert scale, such that you can add multiple rows such as 'First name', 'Last name', 'Streetname', 'Postcode' etc... but in a single survey-type question, rather than spread the different text inputs over 4 different questions.

Hello,

I'm looking to create a form that will allow a user to order any number of parts. This isn't e-commerce, there is no online store it just send someone an email on the other end. To that extent I need two features:

1) Populate the field dynamically from the database - which you guys rocked! Seriously. Awesome documentation.

2) Order an unlimited number of parts. Ideally after selecting one option in the drop down another drop down would appear underneath the first. This would go on until they need no more parts and press submit.

Is there any way to do the second question without manually creating 100 fields and then selectively showing them when the previous drop down is manipulated?

Thanks,

Hello,

I'm using the pre-render filter to get information from Form 1 to populate fields in Form 2. I can get single line text, address fields, and checkboxes, but I'm having trouble getting the list field data to display properly. Can anyone lend a hand?

Thanks.

I wonder if there is a way to do the following:

I have a form that will be filled out by a coach. He will list the players that he is registering - up to ten of them. But the name also needs to have other information, like the following:

Name Reg RC? Which Club

So they would fill in:

Joe Blow Yes RT Rugby Club

Is there a way to set up this sort of thing for ten people? If it was one, I would just set it up with a name, a radio button field, and a text field. But for ten it seems a bit excessive.

Any suggestion would be welcome.

Thanks!

I am wanting to make an all star nomination form and coaches would be able to nominate up to 10 athletes say and I would like to be able to do that from one form and not have to refresh every time. So make multiple entries and then when they are finished one submit sends all the data to a database, is this possible? I see this question already asked but it is in protected content.

Hi,
i am trying to capture the pricing rates of the instructor who opt-in in my list using sign-up form.
i need to capture the following the different rates and store it in mailchimp to allow me to do proper grouping.

Instructor will fill in the Group Rate in the form:
1. Kids - $60
2. Adults - $200
3. Ladies - $240

To do that, I am using advanced field: Lists but the list format as shown below:
a:1:{i:0;a:3:{s:4:"Kids";s:2:"60";s:6:"Adults";s:3:"200";s:6:"Ladies";s:3:"240";}}

however, there isnt a similar list field in mail chimp. how then do i store this information in mailchimp which will allow me to do grouping ?

Pse kindly advise ? or is there a cleaner way to do it ?

best tony

Hello

Is it possible to format the form to resemble a table/excel spreadsheet? Each row would be one entry. The user would then be able to add a row if they ran out. When the form is submitted it submits the form as rows of entries.

Please see below image to demonstrate what im talking about

I am creating a custom HTML email notification and need to separate the list field columns; however, I am unable to find a merge tag that accomplishes this.

My theory was {list:5:1}, which would provide me the value from column one of list ID 5. I've searched the forums and Google for a solution, but have been unsuccessful.

Any help would be greatly appreciated. I've already spent hours on this.
