The only time the date picker shows is when I have a review of one star, then I select "yes" to allow you to make it right. </p> <p>My advice would be to import this file you exported and see if the imported form conditional logic works as expected. Editing a form after creating your conditional logic rules can have unexpected consequences.</p> <p>Additionally, if the form does not display in preview, you could have invalid HTML in one of your HTML blocks, or you have a theme or plugin conflict.
