The form has not been edited and the shortcode has not been changed from [gravityform id=5 title=false description=false ajax=true]</p> <p>Any ideas why it would suddenly disappear and not even show up in the form preview? It used to work just fine.</p> <p>We have updated the plugin to the latest version and are using wordpress 3.5 </p>
