			<description>&#60;p&#62;Not sure what you're talking about here at all. You can't embed a form in a YouTube video. You may be able to embed a screenshot of your form then create a link to your form page, but that's not something that's directly related to this plugin.

			<description>&#60;p&#62;If I have a video that I use with easy video player, creating the elements into the video, form pages appearing at certain time periods, and I upload it to youtube.com, will those things still appear in youtube.com?&#60;/p&#62;
&#60;p&#62;Example:  I have a video that has arrows appearing 5 seconds into the video, a form page that appears at the end of the video, and I upload it to youtube.com, and I embed the youtube code on my website, will that video be what I am expecting with the arrows appearing, form page appearing, etc?
