It's page of catalogs, selected by checkboxes with bcc routing handled with your code here: &#60;a href=&#34;http://pastebin.com/Nyyigyxb&#34; rel=&#34;nofollow&#34;&#62;http://pastebin.com/Nyyigyxb&#60;/a&#62; (thanks for that BTW!).  If we would have had a few more, I would have come up with something.  Duplicating the buttons at the top would be a great option to have for something like this ;)

			<description>&#60;p&#62;There are no settings in Gravity Forms to allow that.  Maybe you can split the first page of the form into multiple pages so the buttons are always visible?  Or maybe use some jQuery to clone the buttons?
