&#60;p&#62;This is possible using Gravity Forms HTML block field. By adding an HTML block field as the first field on your form, you can create a description for your form that fully supports HTML formatting. This will give you the flexibility to add your profile picture to left of the description just like the example you shared.&#60;/p&#62;
&#60;p&#62;Make sure you uncheck the &#34;Display form description&#34; option when you are generating your shortcode for this form since you will be using the HTML block as your form description.
