It's a TITLE field.. not intended to be a place to put an image. You could use the description field - it accepts HTML. You could also use a HTML block element at the very beginning of your form to add your HTML/image tag - those fields are intended for that kind of content.</p> <p>If you want this image BEFORE your form title and the other form content, then why not simply put a paragraph with your image in the page/post content just before your form? I have been unable to find a way to properly do this, if this isn't the correct way. I attached an image.
