			<description>&#60;p&#62;Yeah, adding an image to the form title is gonna break stuff. It's a TITLE field.. not intended to be a place to put an image. You could use the description field - it accepts HTML.  You could also use a HTML block element at the very beginning of your form to add your HTML/image tag - those fields are intended for that kind of content.&#60;/p&#62;
&#60;p&#62;If you want this image BEFORE your form title and the other form content, then why not simply put a paragraph with your image in the page/post content just before your form? That should work too.

			<description>&#60;p&#62;I added an image to the title of my form and it broke the whole thing. I have been unable to find a way to properly do this, if this isn't the correct way. I attached an image. If anyone has a proper solution please share.&#60;/p&#62;
&#60;p&#62;My goal is actually to add a separate &#60;code&#62;&#38;lt;div&#38;gt;&#60;/code&#62; or &#60;code&#62;&#38;lt;span&#38;gt;&#60;/code&#62; to display this image but I haven't been able to find info on that either.&#60;/p&#62;
