&#60;p&#62;Gravity Forms can handle all of that with the exception of limiting the number of checked boxes to 5. This would be readily possible with a little custom jQuery. Gravity Forms supports pricing fields which would enable you to indicate the flat fee to the user. The Gravity Forms PayPal Add-on would allow you to collect that flat fee via PayPal.&#60;/p&#62;
&#60;p&#62;Additionally, in regards to your original question, you can include HTML in the label of each checkbox. This means you could include the &#38;lt;img&#38;gt; tag to display the image next to the checkbox as well. Lots of options! :)

Is this something your forms can do?&#60;/p&#62;
