This would be readily possible with a little custom jQuery. Gravity Forms supports pricing fields which would enable you to indicate the flat fee to the user. The Gravity Forms PayPal Add-on would allow you to collect that flat fee via PayPal.</p> <p>Additionally, in regards to your original question, you can include HTML in the label of each checkbox. This means you could include the &lt;img&gt; tag to display the image next to the checkbox as well.
