There is something going on with W3TC it seems. This morning, as you noted, the stylesheet for the site was not loading at all. So I disabled W3TC and it appeared okay, including the form. But after I reactivated it, it stopped loading the stylesheet again. I clicked a "auto-install" on a dialog box regarding putting in minify code to the .htaccess file and then site was fine, but the form was back to the original problem. Perhaps it's some shared server setting because I'm also getting a warning about minify URL rewriting not working due to server configuration. I see you are running a caching plugin. Would you mind deactivating that to see if it solves the issue.
