This is usually due to a misconfiguration somewhere. Can you change your permalinks to another format, and save them, and then back again, and save again and see if that rectifies the issue? A quick fix would be to edit the confirmation and save it again. It does to a page <a href="http://myURL.com/:/" rel="nofollow">http://myURL.com/:/</a> We have the same website on another URL which works fine, but on this one URL only this form has this error, but the other forms on the same website are fine. What could be causing this?
