			<description>&#60;p&#62;There is no hook for the all_fields functionality.&#60;/p&#62;
&#60;p&#62;But you could always add form # and entry id to your notification email yourself by editing the Notification and then inserting those form field variables.  They would be outside of the HTML table that is created using the all_fields functionality but you could maybe include them below (or above) the all_fields output.
