			<description>&#60;p&#62;I have noticed on more than one form that if I attempt to use my browser/address book to auto-fill my name, address, email, and phone in the form fields they do not fill as I would expect. The addresses city field, zip code field, and the last name field (when using separate first and last name fields) are filled in incorrectly. The city field is filled in with the street address, the zip code field is filled in with the state, and the last name field is left blank.&#60;/p&#62;
&#60;p&#62;Is there a way to get these form fields to accept the correct values?
