You could do this with a drop down for the number of days, then use conditional logic to show that many sections in the form. In each section, you would allow the visitor to choose the number of servings, and make their meal selections.

The email notification can be sent to you for every form submission.

I am looking for an order form plugin for my site http://www.mydiygourmet.com The pricing structure is semi-complicated to me when I have tried to use other similar plugins. I would like to have customer choose the number of days for their meals to be delivered, then choose the number of servings they would like, then pick their meals. Below is the pricing structure (all the meals are the same price) Is this easily done with this plugin?

Once the customer has placed the order, I would also like the order to come to me.

Days
Servings 1 2 3 4 5
1 $50 $60 $70 $80 $90
2 $60 $80 $100 $120 $140
3 $70 $100 $130 $160 $190
4 $80 $120 $160 $200 $240
6 $100 $160 $210 $280 $340

Thanks,
