I tried everything you suggested; deactivating all plug-ins, switching to WP Twenty Ten to see if it is a theme conflict; uploading the plugins one at a time to see if any particular plugin has a conflict, increasing my PHP significantly, and nothing changes. I am unable to add anything after the Form Settings field. I had W3 Cache installed and I deleted it. I cannot understand what the problem is.
