We do need a URL to your form page so we can inspect your form and give you some specific CSS rules to make what you want happen. I am still learning CSS and old HTML habits die hard - when we built forms with tables this was much easier - and yet making any changes was sooooo difficult. Two fields it is... though particularly for widgets having a "name" option in the standard set where first and last name were stacked would be a nice bonus in a future release.
