When you add the checkbox field to the form, do not pre-check the option in the form builder. When the form loads, the box will not be checked. If you check the box, that option (even if there is just one) will be checked when the form displays.</p> <p>If you inadvertently check a radio button selection in the form builder, that will be rendered as pre-selected when the form is displayed, as well. Once you have selected one radio button, you can't ever go back and uncheck all radio buttons in the form builder.
