			<description>&#60;p&#62;Yes Brent, I sent you a response asking for a revised screenshot on the form itself.  I need an example with the form fields filled out because looking at the form with no data populated and the entry screenshot... I don't see what is going wrong.&#60;/p&#62;
&#60;p&#62;I need you to Preview you the form, fill in the form with example data that matches the Entry detail and then take a screenshot of that and send it over to me.

If the Address field was used can you point out which data was entered in which field in the Address field?  So I can recreate this issue on my end exactly as the data was entered on yours.
