			<description>&#60;p&#62;Cool Daniel, no problem. Can you add back in the conditional logic when you get a chance. If you don't want it affecting your live site, maybe just duplicate the form and add at least one condition, then place it on a page that's not linked up on your site and we can take a look about getting it solved.&#60;/p&#62;
&#60;p&#62;It could potentially be a theme or plugin conflict as well.&#60;/p&#62;

Sorry for the incomplete reply, laptop battery died and I'm having to do this via Blackberry.

			<description>&#60;p&#62;Tested on IE 7, 8, and 9; Firefox and Safari, all desktop versions. When my form contains conditional logic, even in its simplest usage, the form won't appear. But as soon as I eliminate the Conditions, the form displays great.
