That's why we have people do our troubleshooting steps first to see if it is a theme or plugin conflict. It seems when you take out the conditional logic, it works normally. What is odd is that if I view the source code presented by the server from the page containing the short code for the form, the form code dose seem to be getting pushed out, it is just not displaying for some reason.
