I see the new test page set up and I do not see a Gravity Form embedded in the page at all. The content div is not showing the output of the shortcode at all. If this problem were due to a JavaScript error, there would be output in the source of the page, but the form would be hidden with CSS because the jQuery to reveal it could not run.
