1. You need to create the form you want.
1.b. Use the "Form" widget and select the form you have created.
2. You need to edit the stylesheet (css) in order to put a border around the form or widget.
3. You can enter this text under Advanced tab "standard value"
4 an 5 could be done within a style sheet (CSS)

Hope this gets you on the right track

Hi Gravity Forum. I am new to this site. I own a small business with a fairly new WordPress Website. I am not a coder or developer (my web developer has suddenly become unavailable to assist me), so I very much hope that you can.

I have created a Quick Request form that I would like to embed in my Empire Woo Theme WordPress Sidebar (the form is not live yet). I am under a tight deadline to test & upload my form, but I have a few questions:
1. I would like to use conditional logic so that the form displays with only a few basic fields in the Sidebar, then once a visitor starts to fill out the form, the form expands (or opens in a new link) so that extra fields appear based upon their previous answer. I would appreciate any instructions & recommendations. I am unfamiliar with using my style sheets & css & scripts as I have only point & clicked/dragged & dropped fields onto my form so far with the form-builder.
2. How can I place a border around the form (as the planned bacground sidebar has a color fade to it & the lower fields of the form look like they are just floating...)?
3. How can I place field labels &/or descriptions inside the form's fields, so the form takes up less (vertical) space in the embedded sidebar form? I hope that when the user clicks on the field, it would over-write the field label/description as they fill in their response.
4. How can I use colored fonts & colour field-fills for my form?
5. How can I add a form title & description in a graphic font & color?

I very much appreciate your assistance with my time-sensitive request. Thank You.
