The plugin automatically outputs the necessary scripts when needed and some of them are placed in the head and others are placed near the footer. I also still see you are running an old version of jQuery in your head. Our forms, when you use conditional logic (which is jQuery based) makes references to it above the footer, but since your theme is loading jQuery in the footer and all of our JS script files the references cannot be made.
