			<description>&#60;p&#62;Searched around to no avail on this one. It's a rather strange client request but here's what they want:&#60;/p&#62;
&#60;p&#62;They will have numerous co-branded pages on their site with a sign up form. They would like to have a notification email go to company partner for the page the form was filled out on. I could of course just make a separate form for each page but then all of the collected information would be in different tables. So I was wondering if anyone has tried to do conditional notifications based on the page the form resides on.
