Ive actually figured out how to do it with the conditional logic. I created a drop down at the top with each of my shoot types. then I created a name entry for each of those shoot types and they only show up if that shoot type is selected. How can I do that? I also need each of these different forms to go to a different mailing list in madmimi. I know how to do that with the madmimi plug in just wondering if it will see each of those forms as a unique form so the name and email of the person will go to the right mailing list </p>
