			<description>&#60;p&#62;How about testing for a theme or plugin conflict.  More likely a plugin that a theme when you are trying to insert using the toolbar button. &#60;/p&#62;

&#60;p&#62;If you try now to reinsert any of your older forms into new pages, using the toolbar button, do they product the same &#34;oops&#34; error?

			<description>&#60;p&#62;Have you exported or imported the forms from another site or server, or changed the domain name?  This error normally occurs when you change domains.  It can also happen if you are missing database tables in your installation.  But if your other forms are working, it's not likely a database problem.&#60;/p&#62;
&#60;p&#62;If you create a new form on the site now, can you insert that?

			<description>&#60;p&#62;On a perfectly working site with already 8 nicely working Gravity Forms, today I got confronted with the &#34;Ooops! We could not locate your form&#34;-issue. I did not change anything to the site and all other forms are working fine - except the new one.&#60;/p&#62;
