This doesn't sound like something you need Gravity Forms for. There are lots of WordPress plugins out there to do just this:

http://www.google.com/search?hl=en&q=site%3Awordpress.org%2Fextend%2Fplugins+%22table+of+contents%22+categories

However, if you're looking to use that information in your forms somehow, please post a followup question. You don't need a Gravity Form to create a page on your site that lists all your categories as you described.

I have several Recipes, each recipe assigned to different categories (and sometimes multiple categories) on this site: http://dalesgrill.com

I'd like to generate a Table of Contents based on the different Categories, and list the recipes that go with each Category on a Page.

Is there a way to create a form that will do that??

thanks very much!
