&#60;p&#62;Yes you can style a form to give it an appearance similar to the example, you would use a placeholder plugin to move the field labels inside the inputs, the hardest part is the file upload field, they are next to impossible to style in such a way that they look the same in any browser, they appear to have created their own fake upload field using div elements which are easier to style.&#60;/p&#62;
