			<description>&#60;p&#62;Where are your visitors entering their middle initial in that form?&#60;/p&#62;
&#60;p&#62;There is no built in name format with the middle initial included.  It's been requested before and is on our list of feature requests.  To get a name input like that, you would have to use three separate fields and style them to make the middle initial narrower and get all three inline.
