When I create the post using the form, it only posts the url of the image and I'm looking for the image to actually display. Doing that will allow you to use Merge Tags to place content in the post body from the other fields and you can also format it in the way you'd like. I created the forms that I wanted and the drop downs, check boxes and others in them. I then placed the form on one of my pages and everything went fine. I then looked at the post it created and it didn't list any of the results from the checkboxes and other things.
