While I have never seen this before and it is not something that has been reported until now, this does indeed look like a bug. Specifically a bug in the Signature Add-On. It's the Signature Field that is causing the issue to occur.</p> <p>If you use the Multi-Select Field and Date Field together the issue does not occur, but once you add the Signature Add-On it causes a jQuery error and as soon as your browser encounters any javascript error it quits executing all javascript.</p> <p>I have replicated this issue locally. I am reporting the issue as a bug and we will take a look and see what needs to be patched in the Signature Add-On in order to resolve this issue. However, this form does not display the Date Picker when clicked, nor does the Multi Select field utilize the enhanced user interface option.
