&#60;p&#62;The removal of that script from your page returned the datepicker to the way it was supposed to look.  Find out how that is being included, and remove it if you're not using the functionality elsewhere, or disable it on a page where you are trying to use the Gravity Forms datepicker.

			<description>&#60;p&#62;Looks like there is a script conflict with the jQuery Datepicker - the containing div has a set width value of &#34;0&#34; which isn't correct. I see that there are some scripted elements on your form (change the color of the form labels on focus, etc) and I would start by looking there.. for any scripts that relate directly to your forms.&#60;/p&#62;
