The HTML fields presented would be determined by what conditional logic was used and the users inputs to previous fields. </p> <p>Then I would guess you could just hide the submit button with some CSS. So that part would have to be custom written.</p> <p>You would create a Gravity Form with the questions you want the user to answer, and then on the Confirmation settings you would redirect to your custom PHP page and pass the form field values.
