But the confirmation text does not appear, it has a disaply:none on the containter div.</p> <p>jquery is loading only once ( I am pretty sure). At the very bottom of your page just before the closing body tag you have a reference to a very old version of jQuery placed. This is what is causing your form not to show up. Up above in the head, you have a reference to a current version, this is correct. Even when the JavaScript works, the display:none is still there, but the JavaScript enables it. This is most likely coming from your theme. Have you tried switching to the Twenty Eleven theme or even previewing the form in the formbuilder preview?
