			<description>&#60;p&#62;We need a form for a shuttle service. The departure drop down lists locations. The destination drop down lists the same locations. We'd like to use the kilometers between each location to calculate the cost of the shuttle at our per kilometer rate. &#60;/p&#62;
&#60;p&#62;The user would select a place from the departure location and a place from the destination location. The form would calculate and show the price of the trip. The user would complete the form and purchase the trip using PayPal. We'd also like to capture the users email with for aWeber.&#60;/p&#62;
