			<description>&#60;p&#62;I have a list of companies with an address, email, url and phone number. I want the user to be able to select the company and once they do it will populate the address, email and phone number fields. How would I do this if the drop down is only the company name and I want the form to look like this once it outputs:&#60;/p&#62;
