MailChimp has a way to generate a simple sign up form in their administration tool. You'd have to find it on their site, they provide a tool for creating simple sign up forms like this one. Or you can use Gravity Forms and the MailChimp Add-On.

What I have done is to use the Notification E-mail of my GF form to submit the Name and E-mail fields to my Aweber list via E-mail.

To do this you have to use the E-mail Parser on Aweber. It works great. As soon as a visitor submits a GF Form on my site his details is captured in my Aweber list and that way you build your newsletter / subscriber list.

The form you're referring to is a simple mailchimp email form. That's not actually a Gravity Form.

That said, you can recreate the layout with a single email field and a little CSS tweaking. You can search the forums for "inline form" etc. and find a few examples, or check out this topic from just a few days ago with more details on how to do that layout.
