Bottom field lets you choose categories (multiple). When this form was a single-page form, everything worked, but when I switched it to be multi-page, it stopped. Chosen script loads, no error on running .chosen on the fields, field IDs match up to jQuery Chosen call too. Not sure what's up, hopefully I can figure this out. </p>
