			<description>&#60;p&#62;Just responded via email.  This is a Headway Leaf issue.  The form works fine when embedded on a page using the shortcode.  It's only having an issue when Headway outputs it as a Leaf.  You will have to get in touch with the Headway team.  I provided details in the email response I sent you.

&#60;p&#62;You are doing things a little strange having the drop down show/hide the product field when you could just use a single product drop down and have it set the price.  I'm not sure if this is what is causing the problem or not.  It could be an issue caused by Headway.&#60;/p&#62;
&#60;p&#62;We will know more once we can login and investigate.&#60;/p&#62;
&#60;p&#62;Is this form running inside a Headway Leaf or was it added via the shortcode in the page content itself?
