			<description>&#60;p&#62;I got the following request today from my client.&#60;/p&#62;
&#60;p&#62;&#34;Need a Credit Application that will be added as a link under the user’s Account Settings. Customers will able to fill out and save the attached interactive PDF on-screen and print out the completed application. Advise is needed to gather data from these forms as a possibility but not a must-have.&#34;&#60;/p&#62;
&#60;p&#62;First thing that came to mind is Gravity Forms.&#60;/p&#62;
&#60;p&#62;I know this does not add much to the topic, but at lest you guys can see its something a lot of people are asking for ;c )&#60;/p&#62;

			<description>&#60;p&#62;@PAWS Due to the nature of the data Word or Notepad isn't ideal.  The data contains multiple values, it's serialized data which means it's like a spreadsheet and not a word document.  This is why export to CSV is the best method.  You can then import the data into Excel, Google Spreadsheet, etc.

			<description>&#60;p&#62;No, that's OK.  I was just wondering how a form submission was being converted into a PDF or DOC.  There are PHP libraries available to create PDFs and I was wondering what was being used on your site to create the DOC.  &#60;/p&#62;

I think you could use the hooks (&#60;a href=&#34;http://www.gravityhelp.com/documentation/page/Gform_post_submission&#34; rel=&#34;nofollow&#34;&#62;gform_post_submission&#60;/a&#62; hook probably) that are built in to Gravity Forms to grab the entry data, and create and mail a PDF (or DOC.)  It would be a customization but it's certainly possible.

&#60;p&#62;Let me back up for a second- the original form was built using Formstack.  I then hired a programmer from elance to build the Formstack form into a custom form that prints to a Word doc.&#60;/p&#62;
&#60;p&#62;Since I'm not well versed in backend code, I'm not following your suggestions about the same  &#34;library&#34; or functionality- in other words, I wouldn't know what to look for.&#60;/p&#62;

			<description>&#60;p&#62;I need this feature too.  My clients are dentists who need their patient forms online.  Currently, we use a form (on a SSL page) where the patient enters their medical history info and the data is submitted via Word doc to a designated email address.&#60;/p&#62;
&#60;p&#62;It would be great if GF could do this w/ PDF or Word.
