Conditional logic does not work as you have it configured. You can't make a field appear based conditionally on the value of the entry for that same field. When the form is rendered, it goes through the conditional logic rules and sets the default state. The default is hidden, then the fields become revealed when there is input which matches your condition. That can't happen in your case because you made this field conditional upon the value entered for this field. Get rid of the one from googleapis and make sure the Gravity Forms jQuery is included in the head, not footer of the page, and I think this will work, unless you have any other jQuery errors.
