Whenever something weird like this comes up, I always check the web server that the domain is hosted on, to see if it's shared hosting and what sort of "neighborhood" the domain is hosted in. Some filters block whole IP ranges or maybe just the one IP based on questionable sites.

You can check the supplychainheros.com domain here:
http://www.yougetsignal.com/tools/web-sites-on-web-server/

Looks like it is on shared hosting and there are a couple of questionable sites there which could cause a filter to block it, but I really doubt it. I would try to get more information from your client regarding the REASON it was blocked, a log message or something.

You can check the IP to see if it's blacklisted too. Maybe the company subscribes to some blacklists for the filter:

http://whatismyipaddress.com/blacklist-check

Hello

My client got blocked by their IT department firewall when trying to submit the form on my site:
http://3pl-survey.supplychainheros.com/

Could this have anything to do with Gravity Forms?
