I did something similar for a community site, but I can't find a page where the functionality is visible right now. It was just as you described: a drop down (that's what I used), check boxes or radio buttons and a text area for comments. I grabbed the logged in user name and display name, and automatically included the embedded page/post so I know what they're talking about.
