			<description>&#60;p&#62;Should be simple. I did something similar for a community site, but I can't find a page where the functionality is visible right now.  It was just as you described: a drop down (that's what I used), check boxes or radio buttons and a text area for comments.  I grabbed the logged in user name and display name, and automatically included the embedded page/post so I know what they're talking about.  It was pretty straightforward.

&#60;p&#62;I'm planning on having a checkbox to chose from a list of items on the page (this be static, doesn't need to be dynamic) and a textarea to specify with comments.&#60;/p&#62;
