A complex form will fail to appear if you have JavaScript or jQuery errors on the page. Here is one error I see right now:

Timestamp: 1/28/2013 11:25:21 AM
Error: ReferenceError: gf_apply_rules is not defined
Source File: http://divorce.avocat-broquet.fr/avocat
Line: 1

The first thing I would do is turn of W3 Total Cache. That is making it difficult to see if jQuery is being included on time, in the head of the page, and of the proper version. Please turn off the caching plugin and post again and we will take a look for JavaScript errors.

Hi,
I created a very complex form (perhaps the cause of this problem...)
It shows up correctly when I launch it in Preview mode. But, when I try to include it in a page... nothing (in the source code of the page, the script of the form appears (http://divorce.avocat-broquet.fr/avocat ).
Please, help me, I don't really understand why it doesn't work.
Many thanks in advance for your assistance.
