You mentioned a few posts back that it does. If that is the case, then the issue is with the theme. Let's stick to one Gravity Forms installation on one domain. Please be sure you are using the latest Gravity Forms 1.6.11 and WordPress 3.5 and post the link to a form on one domain which does not display. We will troubleshoot just one of these and hopefully find a common problem affecting all your sites. It seems like most of them I checked were using the Wellness theme from Themeforest?</p> <p>The issue occurs when you use conditional logic. It's because you have a JavaScript error on the page.
