			<description>&#60;p&#62;I had placed the shortcode inside a div (with blank lines still surrounding it, though), which I'd planned to float next to the embedded map. Removing that div solved the problem, though I have no idea why.

			<description>&#60;p&#62;Same result when all other plugins are disabled, and the form does work in the preview. The problem also exists in Twenty Ten and Twenty Eleven.&#60;/p&#62;
&#60;p&#62;Also tried re-uploading the plugin files, to no avail.
