&#60;p&#62;This is called conditional logic and when a form with conditional logic fails to display, it's typically due to a JQuery or JavaScript error.  Please provide a link to a page with a non-working form and we'll check there for errors which would prevent your form from displaying.  Thank you.

			<description>&#60;p&#62;Without a link to your site it will be more difficult to provide support.  Please provide a link when you are able.&#60;/p&#62;
&#60;p&#62;It sounds like your host is running out of resources though, most likely memory.  Can you try creating a simple form with just a couple fields and see if that works OK?  If it does, I would take a look at the amount of RAM available to both PHP and WordPress.  You may need to contact your host to get that information.

			<description>&#60;p&#62;this isn't a theme or plugging conflict as far as I can tell as the preloaded form will show correctly both on the site and in preview.&#60;/p&#62;
&#60;p&#62;when new forms are previiewed in the creator or in a live site page absolutely nothing shows. not even the form title.&#60;/p&#62;
&#60;p&#62;unfortunately I can't set a page up to example this for you as I am on a mobile device however I will return and do so at my next opportunity
