It seems that the issue is only with the condition "IS NOT", and the problem is not just the form I linked above. The errors are not pointed out below however. Maybe it requires a field that is hidden by conditional logic? This is a fairly new problem since I've successfully submitted the same form in the past (haven't changed anything in the form).
