I don't seem to be using input masks anywhere and there are absolutely no JS errors in the error log. I have just upgraded to the latest version. As soon as I enable conditional logic on a field, the whole form disappears. When I switch it off, it displays again. Also, can you create a quick test form, one page, with conditional logic and embed that into a page so I can look without having to go through three pages.
